Contact Form
Share your details on our contact form to help us learn more about your business and give us a basic sense of the assistance you're looking for.
In-Home Assessment and Recommendations
Within 24 hours of the assessment, you’ll receive a detailed email outlining my findings and recommendations to help you or your loved one age safely at home and reduce hospitalizations.
Onboarding Call
Once the paperwork is out of the way, we proceed with the team onboarding call. In this call, we go over all of the details for communication, software use, your working relationship with your VA so you are ready to hit the ground running!
Free Consultation
It all begins with a free 30–60 minute consultation to discuss your unique needs and circumstances.
Contract and Scheduling
After our call, I’ll send a contract and invoice for services. Once the invoice is paid, we’ll schedule your in-home assessment and I’ll email you a short questionnaire to gather important details in advance.
Review and Implementation
We’ll schedule a follow-up call or Zoom to review everything, answer your questions, and decide next steps. I’ll then provide trusted, vetted referral sources to begin implementing recommended services, typically within one week.
Ongoing Support
If care needs change, I provide updated guidance and resources to help families adjust safely.
Placement Services
If aging in place isn’t possible, my team can assist with placement services locally or nationally, including helping families transition a loved one from Arizona to another state.
